What is Patient Access?

Patient Access is available to all our patients, helping you to take greater control of your health and well-being by increasing online access to the following:

  • Book & Cancel Appointments
  • Patient Registration
  • Order Repeat Prescriptions
  • Update Your Contact Details
  • View a summary of your health records

You have been telling us that you want to be offered more convenience, choice and control in how you access GP services. Increasingly, you also want to be informed and involved in decision about your own care and treatment.

Evidence shows that patients who are informed and involved in their own care have better health outcomes and are less likely to be admitted to hospital.

Please note: Conran Medical Centre is an inclusive practice - which means everyone is welcome. We believe every person has a right to medical care.  Therefore, we do not need to see proof of your address, personal identification or immigration status to register with us.

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

Here at Conran Medical Practice we believe healthcare is a team effort and work in partnership with our staff, to offer our patients a wide range of medical services.

Working within local and national guidelines, Conran Medical Practice provides a confidential, relaxed and friendly service.

New patient registrations:

Please be advised, identification is no longer required when registering as a new patient at the practice.

Please ask at Reception for a new patient registration form.

 

Out of Hours Access:

If you require access to a GP when the Surgery is closed, (Out of Hours), please call 111

The NHS pledges to provide services at a time that's convenient for you.

Outside normal surgery hours you can still phone your GP surgery, but you'll usually be directed to an out-of-hours service. The out-of-hours period is from 6.30pm to 8.00am on weekdays and all day at weekends and on bank holidays.

If it is not an immediate emergency then call NHS 111. NHS 111 is available 24 hours a day, seven days a week. It can provide medical advice and details of the best local service that can provide care. Telephone consultations and triage (an assessment of how urgent your medical problem is) are an important part of all out-of-hours care.

GPs can choose whether to provide 24-hour care for their patients or to transfer responsibility for out-of-hours services to the relevant NHS England Area Team.

Area Teams are responsible for providing a high-quality service for the local population. Some Area Teams provide care themselves. Others provide care through external organisations. This means different areas can have slightly different services.

Types of out-of-hours care

* Out-of-hours cover may include some or all of the services below:

* GPs working in A&E departments or minor injuries units (MIUs)

* teams of healthcare professionals working in primary care centres, A&E departments, MIUs, urgent care centres or NHS walk-in centres

* healthcare professionals (other than doctors) making home visits, following a detailed clinical assessment

ambulance services moving patients to places where they can be seen by a doctor or nurse, to reduce the need for home visits

You can also visit an NHS walk-in centre (WiC), urgent care centre (UCC) or minor injuries units (MIUs). These can provide treatment for minor injuries or illnesses such as cuts, bruises and rashes. They have proved to be a successful complementary service to traditional GP and A&E services. Some NHS WiCs offer access to doctors as well as nurses. However, they are not designed for treating long-term conditions or immediately life-threatening problems. You don't need to be registered and you don't need an appointment. Any member of the public can simply walk in to be seen, regardless of where they are registered. Some offer pre-booked appointments.

Covid vaccinations

People at greatest risk of serious illness from Covid, including care home residents, over 65s and frontline health and social care workers, are eligible for an autumn vaccine booster.

The Covid booster vaccine is given free on the NHS to people who are:

  •     Living in care homes for older adults
  •     Aged 65 years and over
  •     Aged 6 months to 64 years in a clinical risk group
  •     Frontline health and social care workers
  •     Aged 12 to 64 years who are household contacts of people with immunosuppression
  •     Aged 16 to 64 years who are carers.

This is because the risk of severe Covid continues to be strongly associated with increasing age and underlying health conditions.

When is the booster available and how do I book an appointment?

Vaccinations began on 11 September for those most at risk, including adult care home residents and people who are immunosuppressed.

From 18 September, we will start to invite people in priority order of risk and those eligible will be able to book an appointment on the National Booking System.

Across Manchester there are 50 community pharmacies offering the Covid booster vaccinations.

There will also be a number of other vaccination sites across the City offering the booster vaccination including:

  •     The Vallance Medical Centre, Brunswick St, Manchester M13 9UJ
  •     Florence House Medical Practice, Higher Openshaw Primary Care Centre, 1344 Ashton Old Rd, Openshaw, Manchester M11 1JG
  •     The Jolly Medical Centre 74 Crescent Rd, Manchester M8 9NT
  •     121 Withington Rd, Whalley Range, Manchester M16 8EE

How you make a complaint about primary care services is changing on 1 July 2023  

You have the right to make a complaint about any aspect of NHS care, treatment or service, and this is written into the NHS Constitution on GOV.UK. 

From 1 July 2023 the way members of the public make a complaint about primary care services to the commissioner is changing.  

 By primary care services we mean GPs, dentists, opticians or pharmacy services. 

There are two ways you can make a complaint: 

  • You can complain to the healthcare provider: this is the organisation where you received the NHS service, for example a GP surgery or dental surgery.   
  • You can complain to the commissioner of the service: this is the organisation that paid for the service or care you received. 

After 1 July 2023 if you want to make a complaint about primary care services to the commissioner you will now contact Greater Manchester integrated care board instead of NHS England.

If you want to make a complaint directly to the provider of the primary care service, you still can – that does not change on the 1 July 2023.

NHS England’s Customer Contact Centre will still deal with general enquiries from patients, such as ‘how do I get a GP or Dentist’. For such general enquiries, patients can still call 0300 311 22 33.

Members of the public with ongoing complaints received on/after 1 July 2022 will receive a letter from NHS England informing them that the ICB is now handling their complaint with confirmation of their case handler. 

Members of the public with any ongoing complaints received before 1 July 2022 will receive a letter from NHS England informing them that their complaint is being retained by NHS England with confirmation of their case handler.

Conran Medical Practice is leading the way as a Safe Surgery

Conran Medical Practice is proud to be one of the first 500 Doctors of the World Safe Surgeries in the UK, providing accessible healthcare for the most vulnerable members of our community.

Safe Surgeries launched in 2018 and aims to remove barriers to GP registration by providing clinical and non-clinical staff with free training, advice, and resources on how to make their practice inclusive and welcoming for everyone.

It has been endorsed by the Royal College of General Practitioners and the Royal College of Nursing. The initiative gained greater prominence in 2020 as COVID-19 put health and access to healthcare at the forefront of people’s minds, while creating new barriers to GP registration as health services moved online and those without the means or ability to access them were cut off from care.

Things have changed in our Practice, following the Covid-19 Pandemic but, rest assured, our doors are open and our appointments, Face to Face and Telephone assessments, are available to book. Whether that is booked on the telephone, online or in person at Reception.  We are working harder than ever to ensure you receive the high standard of care you have always received from Conran Medical Practice.

Use the NHS App to:

  • get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it
  • order repeat prescriptions - see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to
  • book appointments - search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
  • check your symptoms - search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you
  • view your medical record - securely access your GP medical record, to see information like your allergies and your current and past medicines

Disabled Access:

Disabled access and disabled parking is available at the front of the building.

On line Repeat Prescriptions:

Please allow at least 48 hours before collection excluding bank holidays and weekends.

We no longer take repeat prescriptions over the telephone. 

Named GP: The practice is now required to allocate all patients a named and accountable GP under the terms of the latest GP contract. Invidividual patients will be informed of their named accountable GP at the first appropriate interaction with the patient. If you wish to know who your accountable GP is then please enquire at reception or by giving us a telephone call on the above number.

Collaborative Working:
Conran Medical Centre is currently working with 9 other medical practices in the Blackley, Harpurhey and Charlestown area, as part of a local Primary Care Network, (PCN). The PCN provides a range of shared services to our patients and gives access to the best possible healthcare services.

In January 2021, the PCN established the Covid Vaccination Centre at our Plant Hill Site, providing Covid19 vaccinations for patients across all 9 practices.


Medical Reports/Forms/Private Letters

Our practice has decided to outsource our medical reporting work to an NHS Digital
accredited company called MediData.


MediData will be processing your medical report via eMR and providing online access via
their secure encrypted portal.

LIVI - Online Digital GP Consultation Service

Livi is an online digital GP consultation service, provided via a technological platform in order to increase patient access to GP services; boosting the resilience of participating GP practices by providing additional clinical capacity.

The overall aim of the service is to ensure that the Manchester population has timely access to GP services, provided by qualified healthcare practitioners to meet the increasing demand on GP Practices and shorten patient’s wait time for consultation. This aim will be achieved by utilising a technological digital platform to access healthcare through a computer or smartphone application. This will enable patients to be provided with medical care and advice through confidential video and telephone meetings.
